Prospective students can complete this form to tell us of the support they may require for any disability, Specific Learning Difficulty (SpLD), mental health condition or financial concerns. We will use this information to begin to map out the support you would receive as a student at Cardiff Met.

Confidentiality agreement

Please read this confidentiality agreement - it is important you fully understand your rights about data protection and confidentiality.

This agreement is between you (student) and Student Services. The aim of this agreement is to help you understand what to expect from the Wellbeing Service and what we expect from you.

Confidentiality

Student Services offer a private and confidential service, and we will not share your information outside of the agreed people involved directly with your support and care whilst applying to University. 

We will share anonymised, aggregated data from the question asking you to highlight any worries you may have about attending university and where you plan to live with colleagues in academic schools. No individual will be identifiable in this data.

You can tell us at any time if you do not want us to share any information about your support, this will limit your support options and may directly impact on your support and your studies whilst at the University. If you are concerned in anyway about your information and confidentiality please come and speak to someone in the team and they will go through this with you to fully explain what this means.

Data protection

Data Protection Statement - Under the Data Protection Act 2018 you have a right to access your personal data held within your file.

Although certain information may be exempt from disclosure, you can ask to access this personal data by contacting Cardiff Met's Data Protection Officer at Dataprotection@cardiffmet.ac.uk and further details will be provided for you.

Managing Student Information. - Any information you share regarding your personal circumstances, disability, health or financial situation, will be treated in a confidential and non-judgemental manner.

Retention. – If you choose to attend Cardiff Metropolitan University, the information you provide in this form will become part of your student record on MetHub and will not be shared beyond the Student Services team without prior agreement, unless there are concerns about your safety or the safety of others, or when a court of law legally requires information.

If you do not choose to study at Cardiff Met, the data you provide in this form and any other records of interaction with Student Services will be deleted in the November of the year you originally intended to begin your studies.

We use your information:

  • To co-ordinate any support arrangements with key people involved in your studies, Mental Health and disability support.
  • For monitoring purposes and statistical analysis (this is always anonymous).
  • We keep our records electronically encrypted and stored securely, and all personal information is kept in strict accordance with the Data Protection Act 2018.
