Student Retention Referral Form

No user information will be saved with this form except for any personal details you choose to enter.

Please note that the primary focus of the Student Retention Team in 25/26 will be supporting students through the new Support to Study process. We will be prioritising supporting students who are flagged for engagement concerns through the Metstats system. 

Please note the following:

  • If you have a student who has expressed a wish to change course, withdraw or action Interruption of Studies and are looking to explore their options, please direct them to the Seeking Support Form
  • If the behaviour of a student gives any staff member or fellow student cause for concern, please complete a Cause for Concern
  • If you do not have immediate concerns, but feel the student would still benefit from support, please direct or encourage the student to complete the Disability, Wellbeing and Counselling support form.
  • If you have a student who has low engagement or has failed to submit assessments, they will be picked up automatically by the Mitigating Circumstances and Support to Study procedure.
  • If your concern does not fit into one of the above categories, please continue to complete this form and we will attempt to reach out to the student when we have capacity to do so.

When to complete this form

This form is for academic engagement concerns, or where a student has expressed a wish to withdraw only. We expect 3 separate attempts to communicate with the student before referring to the Retention Team.

If you believe a student is at risk, complete a Cause for Concern.

If they are in need of wellbeing support, refer them to Disability, Wellbeing and Counselling support.

Communication history

We expect at least 2 attempts to communicate with the student over a 2 week period before referring to the Retention Team.
